CHC has had a long history of utilizing technology to provide support to our providers in the form of immunization- and chronic-disease-based registries. Today we have a functioning electronic health record that communicates with pharmacies, labs and hospitals—and now allows patients to have access to critical information. CHC’s clinicians have full access to up-to-the-minute information on their patients—wherever they are. Clinicians can access patient records remotely and send critical information to other health-care providers instantly. This transition has significantly improved the quality, safety, and timeliness of care. CHC is now moving to bring the benefits of the “HIE” or Health Information Exchange to its patients. Those patients who wish to access their own health data can do so through the patient portal.
