1/20/12 COMMUNITY HEALTH CENTER LAUNCHES INNOVATIVE PROGRAM FOR TREATING CHRONIC DISEASES
CHC is one of the first community health centers in the nation to replicate the ECHO model. Project ECHO™ (Extension for Community Healthcare Outcomes) is a model developed by the University of New Mexico initially to extend Hepatitis C care and management to the rural clinics.
12/21/11 COMMUNITY HEALTH CENTER, INC. OPENS SCHOOL-BASED HEALTH CENTER AT SMALLEY ACADEMY IN NEW BRITAIN
The center, located at 175 West Street, is the third that CHC has opened in New Britain and is the newest on the growing list of 38 throughout the state.
12/21/11 HEALTHCARE FOR THE HOMELESS PROGRAM TO HOLD MEMORIAL SERVICE AND CANDLELIGHT VIGIL FOR NATIONAL HOMELESS PERSONS’ MEMORIAL DAY
In recognition of National Homeless Persons’ Memorial Wherever You Are Healthcare for the Homeless (WYA) —a program of Community Health Center, Inc. (CHC)— and St. Vincent de Paul Middletown hosted today a memorial service and candlelight vigil at the Church of the Holy Trinity in Middletown, in memory of the areas’ homeless residents who passed away in 2011.

(CHC) is pleased to announce the receipt of a $50,000 grant from Vertex Pharmaceuticals Inc. in support of the Hep-C Circle of Care program, a pilot competitive grants program designed to foster innovative approaches to patient care and to support people living with chronic hepatitis C.

Courtney — who described himself as an advocate of issues related to nurse practitioner practice — is a strong supporter for amending title XVII of the Social Security Act to remove barriers to access to timely home health care for Medicare enrollees.

8/18/11 New Report Shows Utilizing Telemedicine In Primary Care Settings Can Improve Diabetic Retinopathy Diagnosis Rates for At–Risk Patients
The project, which was conducted in partnership with Community Health Center, Inc. and Yale Eye Center/Department of Ophthalmology, supported telemedicine, using digital imaging technology in the primary care office, as a strategy that can be used to screen underserved and at–risk patients for DR, increase compliance with screening, and streamline specialist referrals.
