Patient Portal Directions

Step One: Registration

Type the following address in your internet address bar:
http://Yourchc.com.

Please save to your favorites so it will always be at your fingertips.

Click the Registration/Forgot Password? link to open the next page:


Fill in your information, including a User Name and Password, and then click the Submit button.

PLEASE NOTE The User Name is not case sensitive. Use the following format: FirstLastXXXX (last four of your Social Security Number). Also, make sure to enter your email on file at CHC and the password you would like to use for the portal.


The next page that comes up is the Consent for Use. You must check the box confirming that you have read the Consent Form and click the Agree button to continue.


Step Two: The Patient Portal Home Page

Below is the home page of the portal. You are reminded that the Patient Portal is for "non-urgent communication only!" Also in the main window you can click on links to access your Personal Health Record (PHR), Messages from CHC, and view Health Maintenance Reminders.


The links on the left-hand side of the screen, allow you to communicate with your CHC provider and provides access to your personal health data, which includes:
Review of Lab, Diagnostic Imaging and Referral Reports; review of your PHR; and the ability to request Medication Refills, Lab Tests and Referrals.


Step Three: Reviewing Your Personal Health Record (A future feature)

To view your Personal Health Record (PHR) use one of the following links in the main window, Click here for PHR - View or Click here for PHR - Complete Report:


Here’s the PHR-View, click each tab to view Allergies, Problems, Vitals, etc.


Step Four: Medication Refill Requests

When you click the Refill Requests link on the left-hand side, your current medication list will be generated.
Check the box for the medication for which you are requesting to have refilled, then click Refill Request button.


Next, verify or select the Pharmacy from which you would like to pick up your medication(s). Complete the Number of refills requested for each medication. Also, include your Contact phone number and any additional Comments if necessary. Click the Send button to complete the process.


Step Five: Lab and Diagnostic Requests

In order to request labs and x-rays, click the Lab Request link and type in your request. When you’re done entering your request, click the Send button to forward it to your Medical Provider.


Step Six: Referral Requests

In order to request a referral, click the Referral Request link and type in your request in the Message window. When you’re done entering your request, click the Send button.


Step Seven: Reviewing Results (Labs/Diagnostics, and Referrals)

The links under the Review section will allow you to look at your Lab/Diagnostic Reports, as well as Referrals.


Click on the lab you would like to view and this screen will open:


Step Eight: Viewing an Upcoming Appointment

Clicking the Current Appointments link under the Appointment section will allow you to look at your upcoming CHC Appointments. The information displayed includes, the Date, Time, and Provider.


Step Nine: Getting Help on the Patient Portal

If you need assistance while using the Patient Portal, click the Help link.


Use the Search field to look up the information needed to assist you:


Step Ten: Signing Out of the Patient Portal

When you are done, click the Sign out link to end your portal session.
