The Community Health Center, Inc. (CHC) is one of the country’s premiere Federally Qualified Health Centers, with a staff of 350 serving 70,000 patients across Connecticut. CHC provides high quality primary medical, dental, behavioral health and social services.
After 30 years, we have learned what works and what does not; recognize what yet needs to be done, and are prepared to take a leadership role in creating systemic improvements.
In 2005, CHC established the Weitzman Center for Innovation in Community Health and Primary Care as a new division devoted to affecting practice and policy through research, publication and consulting. One of our ongoing efforts will be to convene people and institutions. Our 2006 symposium, partially funded by the Aetna Foundation and in collaboration with the Center for Community Partnerships, Wesleyan University, is a first step.
The Center is named in memoriam for CHC Founder, Gerard Weitzman (shown at right), recognizing his deep commitment to the community, to health and to innovation.
