Opened in 1972, CHC of Middletown wants to be the “medical home” for you and your family. The site offers routine and sick care for newborns to grandparents, and has specialty providers that offer you foot care, diabetes care and a nutritionist. It has a variety of programs such as the EDP/Wise women which offers yearly pap smears and mammograms for uninsured women, as well as HIV services. It also offers OB/GYN, prenatal, lactation, family planning, and childbirth education services.

On Tuesdays patients and staff members get together for the “walk and talk” program with our nutritionist strolling along Main Street. A stress reduction program is offered over eight weeks to help relax and be mindful.

The dental site is your “dental home”. The site's dentists and hygienists work with you to keep your teeth and gums healthy and strong through routine and preventive care. If you are over 62-years-old please inquire about our AAA grant program to help supplement the services needed.

The site's Mental Health Department offers care for children, adults, and families as needed, and has social services to help with pharmacy assistance programs, a domestic violence shelter and counseling and support programs.

The site also offers an exciting scheduling system called “open access,” which means you are able to be seen when you want or need to be seen. Just call our office, as an established patient and we will give you an appointment today or within a few days. CHC of Middletown provides:

CHC of Middletown is open Monday through Thursday 8:30 a.m. to 6:30 p.m. and Friday from 8:30 a.m. to 5 p.m. The medical and mental health departments provide emergency after hours call and coverage to established patients.
