Every CHC Patient Has a Team!

CHC provides team-based care. This means our medical providers work with dentists, nurses, counselors, nutritionists and other specialists to offer complete care for every child.

Our promise is to keep your child healthy and happy at the same location, with the same providers and staff, and the same great care for your family!

We use the latest technology to coordinate care and address each child’s unique needs. Our goal is to have long-term, trustworthy relationships with all of our patients. As part of our services:
•We use medical interpreters
•We use electronic health records for all patients, easily viewed by you online
•We can help you apply for state medical insurance
•We have a sliding fee scale - all patients are seen regardless of ability to pay.*

*Anyone can apply for the sliding fee discount; it is based on your income and family size. Please speak with a receptionist or Access to Care worker for assistance with the application. You will need to present proof of income with the application.
