Community Health Center, Inc. Achieves Joint Commission Accreditation For Behavioral Health and Ambulatory Health Care

AWARDED PRIMARY CARE MEDICAL HOME CERTIFICATION

MIDDLETOWN, Conn., April 18, 2018: – Community Health Center, Inc. (CHC) has earned The Joint Commission’s Gold Seal of Approval® for Behavioral Health Care Accreditation and Ambulatory Health Care Accreditation. The Gold Seal of Approval® is a symbol of quality that reflects an organization’s commitment to providing safe and effective care.

CHC also earned the Commission’s Gold Seal of Approval® for Primary Care Medical Home (PCMH) Certification, reflecting its commitment to meeting optional standards for patient-centered care. PCMH Certification is a special add-on option for Joint Commission-accredited ambulatory health care organizations and can potentially result in increased patient satisfaction, improved patient outcomes and reduced overall costs to the health care system.

To achieve the respective three-year accreditations, CHC underwent a rigorous review and four-day onsite survey, including direct observations of clinical operations and staff interviews conducted by a team of Joint Commission surveyors.
