We are also now offering an enhanced, easy-to-read bill statement – designed with you in mind.
InstaMed Patient Payment Portal
To create your new InstaMed Patient Payment account go to pay.instamed.com/chc1.
Use either the Quick code provided on your recent statement (see example of our new statement format below) or your Patient ID which is available on your new statement circled in RED below. Do not use the ID on the detachable coupon (red X).
You cannot obtain your patient ID from an OLDER statement that does not look like the statement below. Please do not use any reference numbers from an old statement to set up your account. Wait until you receive a new statement to set up your account.
The portal will lead you through the enrollment process.
