Eleven CHC locations received the highest level of national recognition as patient-centered medical homes by the National Committee for Quality Assurance. This was the largest number of primary care sites recognized within an organization in Connecticut. CHC has always focused on long-term, collaborative relationships between doctors and patients. These relationships are fostered by coordinated care and technology to meet each patient’s unique needs.
