You will receive a bill from CHC after an appointment at a health center, mobile health unit, on a telehealth visit or at one of our school-based health centers across the state.
CHC accepts many insurance plans and has options if you do not have healthcare coverage. Patients with Husky Health/Medicaid, Medicare, and commercial health insurance are welcome. Check with your insurance company about specific plan details to understand the cost of services. CHC also offers a Sliding Fee Discount Program to help pay for care. You can take advantage of the program whether you have insurance or are currently uninsured.
CHC is here to help connect you to resources available. Our Access to Care Team can walk you through the enrollment steps and see if you qualify for Husky Health/Medicaid or the Sliding Scale Discount Program. They are available to meet in-person or over the phone.
The Sliding Fee Discount Program provides reduced rates for those who qualify. If you qualify, visit fees will be based on a sliding scale using income and household size. Be sure to provide information for all your household members in the application. Documentation like prior year W-2 and recent pay stubs are required as part of the application process. You will be charged full rate for visits until documentation is provided. Patients can take advantage of this program whether you have insurance or are currently uninsured.

There are many factors that will determine how much your visit will cost. Primary care visits are different from dental visits and costs vary. Patients without insurance or who are paying out of pocket for services can request a Good Faith Estimate for total expected costs of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and other fees.
You will receive an account statement in the mail after your appointment. Your statement will reflect any unpaid balances on your account after any insurance coverage you have has been applied.

At your appointment, the Patient Services Associate will verify any insurance coverage you have. If you are playing directly, they can connect you to the resources available including the Sliding Scale Discount Program.
