We take steps to ensure that you are able to afford the quality care that you deserve.

Paying For Care

You will receive a bill from CHC after an appointment at a health center, mobile health unit, on a telehealth visit or at one of our school-based health centers across the state.

CHC accepts many insurance plans and has options if you do not have healthcare coverage. Patients with Husky Health/Medicaid, Medicare, and commercial health insurance are welcome. Check with your insurance company about specific plan details to understand the cost of services.  CHC also offers a Sliding Fee Discount Program to help pay for care.  You can take advantage of the program whether you have insurance or are currently uninsured.

  3. Mail: Payments can be mailed to Community Health Center, Inc., Attn: Patient Accounts, 19 Grand Street, Middletown, CT 06457.  Please include the detachable portion of your statement with payment.

  4. In person: Walk into any CHC location and speak with a Patient Services Associate to pay.

CHC is here to help connect you to resources available.  Our Access to Care Team can walk you through the enrollment steps and see if you qualify for Husky Health/Medicaid or the Sliding Scale Discount Program.  They are available to meet in-person or over the phone. 

Sliding Scale Discount Program Available

The Sliding Fee Discount Program provides reduced rates for those who qualify. If you qualify, visit fees will be based on a sliding scale using income and household size. Be sure to provide information for all your household members in the application. Documentation like prior year W-2 and recent pay stubs are required as part of the application process. You will be charged full rate for visits until documentation is provided.  Patients can take advantage of this program whether you have insurance or are currently uninsured.

How do I apply for the Sliding Scale Discount Program?
  • Click here to complete the application. Documentation is a required part of the application process and demonstrates proof of income. You will be charged full rate for visits until documentation is provided. Patients can take advantage of this program whether you have insurance or are currently uninsured.
  • Accepted documents for Proof of Income:
    • Prior year W-2
    • Two recent pay stubs
    • Letter from employer or Form 4506-T (if W-2 is not filed)
    • Self-employed: Three months of income and expenses for the business
    • If you do not have any written proof of income, you can submit a self-declaration of income letter. Our mailing address is:

How much will my appointment cost?

There are many factors that will determine how much your visit will cost.  Primary care visits are different from dental visits and costs vary.  Patients without insurance or who are paying out of pocket for services can request a Good Faith Estimate for total expected costs of any non-emergency items or services.  This includes related costs like medical tests, prescription drugs, equipment, and other fees.

You will receive an account statement in the mail after your appointment. Your statement will reflect any unpaid balances on your account after any insurance coverage you have has been applied.

My insurance status has changed, can I still come to CHC?

Yes, CHC accepts all patients – including those who have private insurance, Husky Health/Medicaid or plan to pay directly for care. Please contact Patient Accounts to see if we participate in your plan at 888-270-6698 or 860-347-6971 option 6, (extension 4111).

At your appointment, the Patient Services Associate will verify any insurance coverage you have.  If you are playing directly, they can connect you to the resources available including the Sliding Scale Discount Program.

What if I need help paying for prescriptions?

If you have concerns about paying for your prescriptions, we have a Pharmacy Assistance Program that may be able to help. Please talk to your provider about your concerns, and they can connect you with that program.

What other financial assistance is available?

CHC offers payment plans and partial payment options for patients who experience additional financial hardship.  Contact our Patient Accounts/Billing Department at 888-270-6698 or 860-347-6971 option 6, (extension 4111) for more information.
