A Centers for Disease Control and Prevention (CDC)-recognized diabetes lifestyle change program can help you build healthy new habits to lower your risk of type 2 diabetes and improve your overall health.

CHC’s virtual, year-long Lifestyle Change Program includes:

  • A CDC-approved curriculum, including lessons, handouts and other resources
  • A program leader who has been specially trained to help you learn new skills, meet goals, and motivate you
  • A peer support group of other people living with prediabetes who are facing similar challenges and striving for similar goals
