Welcome to Telehealth at Community Health Center

Telehealth visits are a convenient way for you to meet with your provider from the comfort of your home.

For our established patients, we provide video and phone visit options for a variety of conditions and services including Urgent Care, Primary Care and Behavioral Health Care, making it easier for you to meet with your provider when you can’t make it to the office.

How Does It Work?

Seeing a Telehealth provider is quick and easy!

All you need is a video-enabled device and an internet connection.

After you schedule an appointment, we will text you a link to meet with your provider. Click on the link at the time of your appointment to start your visit.

Before your first visit you will also receive a link to read and sign our Telehealth Consent form. Make sure to complete this form before your appointment.
